This is the most popular plan and the one most people qualify for. You answer a short health questionnaire — no invasive medical tests — and if you're approved, you get 100% coverage starting on day one. No waiting period, no reduced benefits in the early years.

The Accelerated Death Benefit rider comes at no extra cost. If you're diagnosed with a terminal illness (12 months or less life expectancy) or confined to a nursing home for 90+ consecutive days, you can access your death benefit early while you're still living. That's a huge deal.

What Health Questions Do They Ask?

For the Level Benefit plan, Mutual of Omaha uses a simplified issue questionnaire — about 10 yes/no health questions plus a height/weight chart. No medical exam. No blood work. No lab visits.

They also run an MIB (Medical Information Bureau) check and a pharmaceutical database check — this is standard and just verifies the information on your application.

Common disqualifying conditions for the Level plan (these would move you to Graded):

  • Currently receiving dialysis, oxygen therapy, or chemotherapy
  • Diagnosed with AIDS/HIV
  • Confined to a hospital or nursing facility
  • Certain organ transplants
