## Medicare in North Carolina: what's different here

Medicare is a federal program, so the basics are the same everywhere. But the plans you can choose from, and how well they fit, depend on where you live in North Carolina.

- **Plans are sold by county.** Medicare Advantage and Part D options vary by county. A plan your neighbor in Wake County loves may not be offered in New Hanover or Buncombe.
- **Networks follow health systems.** North Carolina care is organized around large health systems such as Atrium Health, Duke Health, ECU Health, Novant Health, UNC Health and WakeMed. Before choosing a Medicare Advantage plan, it's worth confirming your doctors and hospitals are in its network.
- **No Medigap birthday rule.** North Carolina doesn't give a yearly window to switch Medigap plans without health questions. Outside your Medigap Open Enrollment Period or a guaranteed issue right, a new Medigap policy usually requires medical underwriting.
- **Retirees and veterans have extra options to coordinate.** Retiree coverage, TRICARE For Life and VA benefits can change which Medicare choices make sense. We'll look at what you already have before suggesting anything.

## Medicare Advantage or Medicare Supplement?

### Medicare Advantage (Part C)

Replaces how you get Original Medicare benefits. Usually includes drug coverage, uses a network, and has copays plus a yearly in-network out-of-pocket limit (no more than $9,850 in 2027). Many plans have low or no extra premium beyond Part B.

### Original Medicare + Medigap + Part D

Works with any doctor or hospital nationwide that accepts Medicare. You pay a monthly Medigap premium and add a separate Part D drug plan, in exchange for more predictable out-of-pocket costs.

Neither is right for everyone. Your doctors, prescriptions, travel plans, health and budget all matter, and in North Carolina the Medigap decision is hard to undo later. That's the conversation we have on a free call.

## Key Medicare dates

| Enrollment period | When | What you can do |
| --- | --- | --- |
| **Initial Enrollment** | 7 months around your 65th birthday | Sign up for Parts A and B, and choose a Medicare Advantage or Part D plan |
| **Medigap Open Enrollment** | 6 months, starting the month you're 65 or older and have Part B | Buy any Medigap policy sold in NC. Insurers can't deny you or charge more because of your health |
| **Annual Enrollment** | October 15 to December 7 | Join, switch or drop Medicare Advantage and Part D plans for January 1 |
| **Medicare Advantage Open Enrollment** | January 1 to March 31 | If you're in a Medicare Advantage plan, switch once to another one or go back to Original Medicare |
| **Special Enrollment Periods** | After certain life events | Moving, losing employer coverage, qualifying for Extra Help or Medicaid, and more |

## What's new for 2027

- **Part D out-of-pocket cap:** $2,400 for covered drugs in 2027.
- **Part D deductible:** no more than $700 in 2027.
- **Medicare Advantage limit:** in-network out-of-pocket maximums can be no more than $9,850 in 2027.
- **Part A and B premiums for 2027:** Medicare usually announces these in the fall. We'll update this page when they're published.

What does an independent Medicare insurance agency do?

An independent agency is appointed with multiple insurance companies instead of just one. Triangle Life and Health compares Medicare Advantage, Medicare Supplement (Medigap) and Part D plans from the carriers we represent, checks your doctors and prescriptions against each plan, and helps you enroll. We do not offer every plan available in your area.

Does it cost more to use a Medicare agency instead of going direct?

No. Medicare Advantage and Part D premiums are the same whether you enroll through an agent, directly with the insurance company or on Medicare.gov, and Medigap rates are filed with the state. Agents are paid by the insurance company, so there is no fee to you.

Do you only work with people in Raleigh?

No. Our office is in Raleigh, but we help people across North Carolina, from the mountains to the coast, by phone and video. If you're near the Triangle, you're welcome to meet in person at our Six Forks Road office.

When can I enroll in or change a Medicare plan?

Most people first enroll during their Initial Enrollment Period, the 7 months around their 65th birthday. After that, Annual Enrollment runs October 15 to December 7 each year, and people already in a Medicare Advantage plan get one more change from January 1 to March 31. Some life events, like moving or losing employer coverage, open a Special Enrollment Period.

Does North Carolina have a Medigap birthday rule?

No. North Carolina does not have a birthday rule, so outside your Medigap Open Enrollment Period or a guaranteed issue situation, switching Medigap plans usually means answering health questions. That makes your first Medigap decision at 65 especially important.
