# Turning 65 in North Carolina: Your Medicare Checklist

Turning 65 comes with a lot of mail and a lot of deadlines. The good news: if you take it step by step, Medicare is very manageable. This checklist walks through what to do and when, with the details that are specific to North Carolina.

## Step 1: Know your Initial Enrollment Period

Your **Initial Enrollment Period (IEP)** is 7 months long: the 3 months before the month you turn 65, your birthday month, and the 3 months after. If you sign up before your birthday month, coverage starts the first day of your birthday month. If you sign up during your birthday month or later, coverage starts the first day of the month after you sign up. (If your birthday is on the 1st, everything shifts one month earlier.)

**Already collecting Social Security?** You'll usually be enrolled in Parts A and B automatically, and your Medicare card arrives about 3 months before your birthday. **Not collecting yet?** You sign up yourself at [SSA.gov](https://www.ssa.gov/medicare), by phone, or at a Social Security office.

## Step 2: Decide about Part B if you're still working

Getting this step wrong can lead to lifelong penalties and coverage gaps. The rule of thumb:

- **Employer with 20+ employees** and you (or your spouse) are actively working: you can usually delay Part B without penalty. When the job or coverage ends, you get an 8-month Special Enrollment Period.
- **Employer with fewer than 20 employees:** Medicare usually pays first, so you generally need Part B at 65.
- **COBRA, retiree coverage or a Marketplace plan** don't let you delay Part B without risking a penalty.
- **Contributing to an HSA?** Stop contributions before Medicare starts. Part A can be backdated up to 6 months when you enroll after 65.

Always ask your employer's benefits office how their plan works with Medicare before you decide.

## Step 3: Understand what Original Medicare costs (2026)

| Cost | 2026 amount |
| --- | --- |
| Part A premium | $0 for most people (if you or your spouse worked about 10 years) |
| Part A hospital deductible | $1,736 per benefit period |
| Part B standard premium | $202.90 a month (higher with higher income) |
| Part B deductible | $283 a year, then usually 20% coinsurance |

**About Part B:** CMS usually announces next year's Part B premium and deductible in the fall. Until it does, any 2027 Part B number you see is an estimate. We'll update this page once CMS releases the official figures.

Original Medicare has no yearly out-of-pocket limit and doesn't cover most prescriptions, which is why most people add more coverage in Step 4.

## Step 4: Choose how you'll fill the gaps

You have two main paths:

|  | Medicare Advantage (Part C) | Original Medicare + Medigap + Part D |
| --- | --- | --- |
| Doctors | Plan network (HMO or PPO) | Any provider nationwide that accepts Medicare |
| Premium | Many plans have low or no premium beyond Part B | Monthly Medigap premium + Part D premium |
| Out-of-pocket | Copays, with a yearly in-network limit (up to $9,850 in 2027) | More predictable, depending on the Medigap plan |
| Drug coverage | Usually included | Separate Part D plan |

In North Carolina, networks are built around health systems like Atrium Health, Duke Health, ECU Health, Novant Health, UNC Health and WakeMed, so check that your doctors and hospitals are in a plan's network before choosing it. ## Step 5: Don't miss your Medigap Open Enrollment window

Your **Medigap Open Enrollment Period** is 6 months, starting the first month you're 65 or older and have Part B. During it, you can buy any Medigap policy sold in North Carolina, and insurers can't deny you or charge you more because of your health.

**The North Carolina catch:** NC has **no Medigap birthday rule**. After this window closes, switching or buying a Medigap policy usually requires medical underwriting unless you have a guaranteed issue right. If you think you might ever want Medigap, the decision you make at 65 matters a lot.

## Step 6: Avoid the late enrollment penalties

- **Part B:** 10% added to your premium for each full 12-month period you could have had Part B but didn't, usually for as long as you have Part B.
- **Part D:** 1% of the national base premium ($38.99 in 2026, $41.33 in 2027) for each full month after your Initial Enrollment Period that you went without Part D or other creditable drug coverage (if the gap lasted 63 days or more in a row), for as long as you have Part D.

## Your turning-65 timeline

| When | What to do |
| --- | --- |
| 6 months before 65 | Talk with your employer about how its plan works with Medicare. If you have an HSA, plan when to stop contributions (see the FAQ below). |
| 3 months before 65 | IEP opens. Sign up for Parts A and B (or confirm automatic enrollment). Start comparing plans. |
| 1–2 months before 65 | Choose Medicare Advantage or Medigap + Part D, and enroll so coverage starts on time. |
| Birthday month | Coverage starts if you enrolled early. Carry your new cards. |
| Each fall | Review your plan during Annual Enrollment (October 15 to December 7). |

## Frequently Asked Questions

### Will I be enrolled in Medicare automatically at 65?

If you're already getting Social Security retirement benefits at least 4 months before you turn 65, you'll usually be enrolled in Parts A and B automatically and your card arrives in the mail. If you aren't collecting Social Security yet, you need to sign up yourself through Social Security.

### Can I delay Part B if I'm still working?

Often, yes. If you (or your spouse) are actively working and covered by an employer plan from a company with 20 or more employees, you can usually delay Part B without a penalty and sign up later during an 8-month Special Enrollment Period. COBRA and retiree coverage don't count. If the employer has fewer than 20 employees, Medicare usually pays first, so you generally need Part B at 65.

### Does North Carolina have a Medigap birthday rule?

### I contribute to an HSA. What changes at 65?

You can't contribute to an HSA once any part of Medicare starts. If you sign up for Part A after 65, it can be backdated up to 6 months (but not before the month you turned 65), so many people stop HSA contributions about 6 months before they plan to enroll.
