Many people move to North Carolina to retire or to be closer to family. If you have Medicare, a move can affect your coverage, and the timing matters. Here's what happens to each type of coverage and what to do before and after you move.

## Quick answer by coverage type

| Your coverage | What happens when you move to NC |
| --- | --- |
| Original Medicare (Parts A and B) | Nothing changes. It works anywhere in the U.S. Update your address with Social Security. |
| Medigap (Medicare Supplement) | You can generally keep your policy as long as you stay with Original Medicare. Your premium may change. |
| Medicare Advantage | Plans are local. If you move out of the service area, you'll need a new plan. |
| Part D drug plan | Many are regional. If you move out of the plan's area, you'll need a new one. |
| Medicaid / Medicare Savings Program | Doesn't transfer. Apply again with NC Medicaid. |

## Your Special Enrollment Period for moving

When you move out of your plan's service area, you get a **Special Enrollment Period (SEP)** to join a new Medicare Advantage or Part D plan:

- **If you tell your plan before you move:** your SEP starts the month before the month you move and continues for 2 full months after you move.
- **If you tell your plan after you move:** your SEP starts the month you tell your plan, plus 2 more full months.

Telling your plan early gives you the most time. New coverage can't start before your move date and always starts on the first of a month, so the earliest is usually the first of the month after you move. If you don't choose a new plan, you'll be disenrolled and returned to Original Medicare, possibly without drug coverage.

## A move can be a chance to get Medigap

If you're in a Medicare Advantage plan and move out of its service area, you have a **guaranteed issue right** to buy certain Medigap policies without health questions, if you apply within the time limit (generally no later than 63 days after your Medicare Advantage coverage ends). That's worth knowing because **North Carolina has no Medigap birthday rule**. Outside your Medigap Open Enrollment Period or a guaranteed issue right, buying a Medigap policy here usually requires medical underwriting.

Already have Medigap? You can keep it after you move. If you want to change to a policy sold in North Carolina, you may have to answer health questions unless you qualify for a guaranteed issue right. (If you have a Medicare SELECT policy and move out of its area, you have a guaranteed right to switch to certain Medigap plans sold in NC.)

## Choosing a plan in North Carolina

Medicare Advantage and Part D options vary by county, so the plans available in Wake County may be different from those in New Hanover, Mecklenburg or Buncombe. Before you choose:

1. **Find your new doctors first.** NC networks are organized around health systems such as Atrium Health, Duke Health, ECU Health, Novant Health, UNC Health and WakeMed. Make sure the doctors and hospitals you want are in the plan's network.
2. **Check your prescriptions and pharmacy** against each plan's drug list.
3. **Compare total yearly cost**, not just the premium: deductibles, copays and the out-of-pocket limit.
4. **Time it with the move** so there's no gap in coverage.

## Moving checklist

| When | What to do |
| --- | --- |
| 1–2 months before | Tell your current plan your move date. Start comparing NC plans in your new county. |
| Before you arrive | Enroll in a new Medicare Advantage or Part D plan so there's little or no gap. Coverage can start the first of the month after you move. |
| After you move | Update your address with Social Security. Apply for NC Medicaid or a Medicare Savings Program if you had one before. |
| First few weeks | Establish care with new doctors and transfer prescriptions to an in-network pharmacy. |

## Frequently Asked Questions

### Do I have to change my Medicare when I move to North Carolina?

It depends on your coverage. Original Medicare works anywhere in the U.S., and you can generally keep a Medigap policy when you move. Medicare Advantage and Part D plans are tied to a service area, so if you move out of yours you'll need a new plan.

### How long do I have to pick a new plan after moving?

If you tell your plan before you move, your Special Enrollment Period starts the month before you move and lasts 2 full months after the move. If you tell your plan after you move, it starts the month you tell them and lasts 2 more full months.

### What happens if I don't choose a new Medicare Advantage plan?

If you move out of your plan's service area and don't choose a new plan, you'll be disenrolled and returned to Original Medicare. You could lose drug coverage, which can lead to a Part D late enrollment penalty later.

### Can I switch from Medicare Advantage to Medigap when I move?

If you move out of your Medicare Advantage plan's service area, you have a guaranteed issue right to buy certain Medigap plans without health questions, as long as you apply within the time limit. That matters in North Carolina, which has no Medigap birthday rule.

### Does my Medicaid or Medicare Savings Program move with me?

No. Medicaid and Medicare Savings Programs are run by each state. You'll need to apply with NC Medicaid after you move.
